Who we are

This website is operated by Dr Gregory Tarr, interventional radiologist (Tarr Radiology), practising in Brisbane and Ipswich, Queensland. In this policy, "we", "us" and "our" mean Dr Gregory Tarr and his practice. We are committed to protecting your privacy and handling your personal and health information in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs) and other applicable Australian laws.

What this policy covers

This policy explains what information we collect, why we collect it, how we use and protect it, where it is stored, and how you can access or correct it. It applies to this website, the online enquiry and referral forms, and the interactive symptom-score tools. It does not change your rights under law or the obligations we owe you as a patient.

The symptom-score tools — collection notice

This website offers interactive questionnaires (for example, symptom or bleeding scores) that help you understand your symptoms and provide a baseline before or after treatment. How your information is handled with these tools is specific and important:

• Your answers stay in your browser. As you complete a questionnaire, your answers and your score remain on your own device. They are not transmitted to us and are not stored on any server.
• The calculator never sends anything. If you would like us to see your score, you can save it as a PDF and email it to us, or print it and bring it to your appointment. That choice is entirely yours.
• If you do send them, your answers and score are treated as health information under the Privacy Act 1988 (Cth) and the Australian Privacy Principles, and are kept in our clinical records.
• Please do not enter identifying details such as your date of birth, address, Medicare number or referring doctor into a questionnaire field. If you contact us, we will collect those details securely through the appropriate channel.
• Analytics never receive clinical information. We measure only how the tools are used — for example whether a questionnaire was started, completed or abandoned, and how long it took. Your individual answers and scores are never sent to analytics or advertising services, and these pages carry no advertising or remarketing pixels.

Information we collect

Depending on how you interact with us, we may collect:

• Contact and enquiry information you provide through the enquiry form or by phone or email — such as your name, phone number, email and the content of your message.
• Referral and health information provided by you or your referring practitioner, including any referral document you upload or send.
• Symptom-score information only if you choose to email or bring in a summary yourself, as described above.
• Technical and usage information collected automatically when you visit, such as anonymised, aggregated analytics about page visits and site performance. This does not identify you.

How we use your information

We use your information to respond to your enquiry, to arrange and provide your care, to communicate with your referring doctor and other treating practitioners involved in your care, to meet our administrative, billing and legal obligations, and to improve this website. We do not sell your personal information, and we do not use your health information for advertising.

When we disclose information

We may disclose your information to others involved in your care (such as your referring GP or specialist, the hospital where your procedure is performed, pathology and imaging providers, and anaesthetists), to Medicare and your private health fund for claiming and billing, and to others where you have consented or where we are required or authorised by law. Where we use third-party service providers (for example, secure hosting or email), we take reasonable steps to ensure they protect your information.

Storage and security

We take reasonable steps to protect your information from misuse, loss and unauthorised access, modification or disclosure. Clinical records are held in secure systems with access limited to those who need it for your care. Where information is transmitted (for example, a referral you send us), we use secure methods and act to protect it in transit and at rest.

Overseas storage and disclosure

This website is hosted by Netlify, Inc., which operates servers in the United States. Information you submit through this website — the enquiry and referral form, including any referral document or symptom-score summary you attach — is transmitted over an encrypted connection and held by Netlify in the United States until we retrieve it and file it in our clinical records, which are held in Australia.

We take reasonable steps to ensure our hosting provider protects your information. You should be aware that once information is held overseas it may be subject to the laws of that country, and that the protections in Australian Privacy Principle 8.1 may not apply in the same way. By submitting a form on this website you consent to this overseas disclosure.

If you would prefer your information not to be stored overseas, please phone the rooms on 07 3107 4875, or send your referral through Medical-Objects instead of using the website form.

Analytics and cookies

This site uses privacy-preserving analytics to understand how the site is used and to improve it. On the symptom-score pages, analytics record interaction events only (such as a questionnaire being started or completed) and never your answers, scores or any identifier. We do not use advertising or remarketing pixels on the symptom-score pages.

Accessing and correcting your information

You may request access to the personal information we hold about you, and ask us to correct it if it is inaccurate, out of date or incomplete. To make a request, contact us using the details below. We will respond within a reasonable time and in accordance with the Privacy Act.

Complaints

If you have a concern about how we have handled your personal information, please contact us first so we can try to resolve it. If you are not satisfied, you may contact the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au.

Changes to this policy

We may update this policy from time to time. The current version is always available at this page, with the "last updated" date shown above.
